AmeriHealth Caritas DC, a health insurer and District of Columbia Managed Care Organization (MCO), offers a full range of services for the DC Healthy Families Medicaid program and the DC Healthcare Alliance. AmeriHealth Caritas DC offers diabetes prevention and support to its health plan members living with diabetes, or at risk of developing diabetes. Howard University Hospital offers individual and group Diabetes Self-Management Education courses for people who have Type 1, Type 2, and Gestational Diabetes. Joslin Diabetes Center, an affiliate of Providence Hospital, offers Diabetes Education Self-Management courses and Diabetes Prevention Program courses. There are three options available for persons interested in enrolling in Joslin Diabetes Center’s courses: Diabetes Today is a three session, comprehensive Diabetes Self-Management Education course. Diabetes Short is a one-time, hour long Diabetes Self-Management Education course. Diabetes Update is a one time, two-hour long, comprehensive Diabetes Self-Management Course.  Joslin Diabetes Center also offers Diabetes Prevention Program courses, personal 1-on-1 education, and a Diabetes support group. MedStar Family Choice is a Managed Care Organization (MCO) serving the District of Columbia. In the District, they provide services to individuals eligible for the DC Healthy Families Program and the DC Healthcare Alliance Program.  MedStar Family Choice offers diabetes prevention and education to its health plan members living with diabetes, or at risk of developing diabetes. Disease Management is available to members who need help managing chronic diseases, including diabetes. The Disease Management service is provided by nurses and social workers who work closely with members and their doctors. They contact members by telephone and help members to understand their diabetes and to get the testing and care they need. They work closely with members to help them learn how to manage their diabetes. MedStar Georgetown University Hospital offers group Diabetes Self-Management Education courses and individual Diabetes education sessions. Both require a doctor’s referral and insurance. The three session Diabetes Self-Management Education courses are held on the 1st, 2nd, and 3rd Thursday of each month from 8 a.m. – 10 a.m. Persons interested in participating in individual sessions with a nurse and dietician must call to make an appointment, after receiving a doctor’s referral. Trusted Health Plan (THP) is a Managed Care Organization (MCO) serving the District of Columbia. In the District, they provide services to individuals eligible for the DC Healthy Families Program and the DC Healthcare Alliance Program.  THP has a Diabetes Disease Management Program. All THP enrollees aged 18+ with a primary or secondary diagnosis of Diabetes or Pre-Diabetes (with the exception of Gestational Diabetes) is eligible to receive the benefits of this program.  All eligible enrollees will undergo a Diabetes Health Risk assessment and receive education and services customized to their severity of illness.
Benefits of this program include:

  • Disease Management Staff Outreach
  • Educational Materials
  • Educational Seminars and Services at the THP Health and Wellness Outreach Center
  • Medication and Preventative Health Monitoring
  • Case Management Services and primary care outreach and care coordination are also available to High Risk enrollees
