Take Action to Live Well

The Living Well Self-Management Programs are evidence based six-week workshops for adults who live in the District of Columbia with one or more on-going health conditions. Each workshop is 2 ½ hours once a week and held in convenient locations in the community. People with different chronic conditions and cancer, as well as those who support them, can attend the workshops. Workshops are facilitated by two trained leaders, one or both of whom are non-health professionals with a chronic condition or cancer.

The DC Department of Health in partnership with our community organization offers:

  • Chronic Disease Self-Management Program
  • Diabetes Self-Management Program
  • Cancer: Thriving and Surviving
  • Tomando de Su Salud

If you or someone you care for has an ongoing health condition such as diabetes, obesity, heart disease, high blood pressure, chronic pain, depression anxiety or cancer, the Living Well Workshops can help.
