National Diabetes Prevention Program (NDPP) was developed by the Centers for Disease Control and Prevention to help people with pre-diabetes change their lifestyle to decrease their risk of getting diabetes. NDPP is offered by workplaces, hospitals and health centers.

Diabetes Self-Management Education & Training Programs (DSME/T) help people diagnosed with diabetes acquire knowledge and skill to navigate decisions and activities that improve health outcomes. Diabetes care is complex and involves many healthcare professionals however DSME is a resource used to address the challenges of ongoing self-management. DSME/T programs are reimbursed by Medicare and many insurance plans.

Chronic Conditions Self-Management Programs are evidence-based workshops for adults with one or more on-going health conditions. The classes are held in the community and help participants manage their conditions using brainstorming, action planning and problem solving.
